POSITION TITLE: Licensed Practical Nurse (LPN)


POSITION SUMMARY: Oversees and coordinates nursing and treatment services for the region.

QUALIFICATIONS: Licensed Practical Nurse or Registered Nurse with one (1) year of experience , preferably in chemical dependency or psychiatric nursing. One (1) year experience in supervisory or coordinator role preferred. Valid drivers license and must be at least 21 years of age to transport clients in agency vehicle.

SKILL SETS: Adequate communication skills, process verbal and written instructions, dependable, interact effectively with the public and staff, adaptability-able to deal with frequent change, delays or unexpected events, strong sense of commitment.

PHYSICAL AND ENVIRONMENTAL DEMANDS: Talking and hearing required to communicate with client, family members, staff and referral sources. Visual acuity needed to provide behavioral observations of clients. Work is light with some walking, standing and sitting. Manual dexterity needed in performing nursing care. Possible exposure to bloodborne pathogens, hazardous and biohazardous waste. Possible exposure to infections and communicable diseases.


  1. Administers injections for clients, maintaining appropriate timely schedules.
  2. Assists in determining medical situations that require referral or medical intervention.
  3. Manages medical supplies/equipment and ensures proper storage procedures are followed.
  4. Coordinates, monitors and evaluates client self administration of medications process.
  5. Assures proper maintenance and documentation of medical records concerning medication and nursing services.
  6. Provides routine and emergency nursing care.
  7. Monitors all nursing care aspects of treatment.
  8. Monitors and performs nursing practices associated with Tuberculosis policies and procedures for clients.
  9. Oversees treatment services, client care and other staff in emergencies.
  10. Ensures compliance with all infection control practices
  11. Reviews data concerning health status of clients, consults with Supervisor and intervenes as indicated.
  12. Provides education and counseling to clients and staff
  13. Monitors medication samples.
  14. Participates in agency continuing quality improvement (CQI) activities.
  15. Performs other duties as assigned.


  1. Adheres to Communicare policies and procedures.
  2. Attends mandatory inservice and other training/educational programs as required.
  3. First Aid and CPR certification.
