POSITION SUMMARY: Participates in 24/7 support to our communities active military personnel, veterans, military family members and their dependents. Coordinates referrals for service member, veterans, and their families. Provides support to individuals experiencing a behavioral health crisis. Serves as a liaison for affiliate agencies, programs and facilities to include military base(s), Veterans Administration programs, and private hospitals and other programs supporting the target population. Completes data entry necessary to support grants and agency contractual obligations.

QUALIFICATIONS: Bachelors Degree in human services or related field and at least one year of work experience in human services. United States Military Veteran with at least (4) years of service with a Honorable discharge. At least (21) years of age. Lived experience with recovery preferred but not necessary.

SKILL SETS: Organizational and adequate communication skills, process verbal and written instructions, computer proficiency, methodical approach to problem solving, work under stress and tight deadlines, interact effectively with the public and staff, adaptability-able to deal with frequent change, delays or unexpected events.

PHYSICAL AND ENVIRONMENTAL DEMANDS: Visual acuity necessary for updating files and completing documents. Hearing and talking is necessary to answer phone and talk with employees and other individuals. Manual dexterity is required to perform work processing and other clerical duties.


  1. Participates in 24/7 crisis on call programs providing support to SMVF population to include response and consultation to Crisis/Suicide hotlines.
  2. Provides face to face contact as needed for crisis situations in an individual's home, crisis stabilization units, hospital emergency departments, and/or the service region at large.
  3. Assists individuals, families, guardians with access to behavioral services.
  4. Interface with affiliate agencies, individual contractors at large and develop and oversee MOUs/agreements/contracts to provide placement options for individuals in crisis.
  5. Attend and make positive contributions to person-centered plan meetings.
  6. Attend meetings with other SMVF-Coordinators and peers (from other participating community mental health centers-CMHCs) with Department of Behavioral Health-DBH staff to discuss information relevant to this role; and provide information, as requested, regarding services available within the CMHC jurisdiction.
  7. Track, prepare and submit required documentation and records for expenditures.
  8. Will prepare and submit DBH report and provide and enter data to grant organization via email or website in a timely manner.
  1. Participate in agency Continuing Quality Improvement (CQI) activities.
  2. Performs other duties as assigned.

  3. Completes and maintains CPR and First Aid Certification.
